I. New Applicant Forms

II. Master's Program Forms - For Current Students

General

  2. Master Program Checklist - Extracted from the student handbook, use a guide the throughout your master's program.

Course Related

  1. Master's Course Requirements Waiver Request / CMS Core Class Waiver Request
  2. Graduate Transfer Course * - Use to officially transfer courses from non-degree or incomplete degrees into your current program, to be completed as soon as possible after entering the program, submit to CMS Academic Affairs office.
  3. Graduate School Petition * - Use to adjust registration after the drop/add deadline, submit to the CMS Academic Affairs office for processing.

 

Thesis Related

  1. Thesis Proposal - SACS Evaluation Form * - Distribute to each of your committee members during your thesis proposal, submit completed forms to CMS Academic Affairs office directly after thesis proposal, target: year 2.
  2. Committee Appointment* - Use to officially form your committee, submit completed forms to CMS Academic Affairs office, target: year 2. 
  3. Change of Committee* - Use to make changes to your committee.
  4. Electronic Thesis and Dissertation (ETD) Resource Center - Review the Graduate School ETD website one semester before graduation.
  5. Thesis and Dissertation (ETD) Workshop (REQUIRED) - Attend an ETD workshop one semester before graduation or complete online assessment.
  6. Application to Graduate Form - Students who plan to graduate must complete and submit their Graduation Application online. Please give yourself an earlier deadline to address any potential obstacles. When you are ready to begin the survey and apply, login into OASIS using your Net ID and self-assigned password, then click on the Student menu and select Apply for Graduation.
  7. Electronic Thesis (ETD) Registration - Complete this Graduate School online form early in the semester in which you plan to graduate, refer to the ETD resource center for deadlines.
  8. Request for Master's Thesis Defense Form* - Submit completed form to CMS Academic Affairs office two weeks before your defense.
  9. Thesis Defense Announcement
  10. Thesis Defense Announcement - with CoMajor - Email a copy of your announcement to CMS Academic Affairs office to Brittany Sheehy two weeks before your defense.
  11. Successful Defense of Master's Thesis Form* - Print a copy and bring to your defense, return signed form to CMS Academic Affairs office directly after successful defense.
  12. Thesis Defense - SACS Evaluation Form* - Distribute to each committee member before your defense, return completed forms to CMS Academic Affairs office directly after successful defense.
  13. Thesis (written) SACS Evaluation Form* - Distribute to each committee member before your defense, return completed forms to CMS Academic Affairs office.
  14. Electronic Thesis - Final Submission Forms. *  NOTE: Our college has a special arrangement with the Graduate School so you do NOT have to drive to Tampa to submit your forms, instead you will deliver all signed documents directly to Brittany Sheehy on or before the posted deadline by 3pm. The final submission process involves uploading your final written thesis to the Proquest website, completing surveys, and hand delivering paper forms in person to Brittany Sheehy. Please review the Master's ETD Submission Benchmarks and Checklists for complete details and ETD deadlines.
  • Certificate of Approval Form * - Requires signatures of all of your committee members and Dr. David Naar. Refer to the ETD website for deadlines. Submit signed forms directly to Brittany Sheehy.
  • USF Proquest (online upload website) - Register and upload your completed thesis in .pdf format to the USF ProQuest website for review and approval by the USF Graduate School ETD Office.

* Multiple signatures of approval are required to complete these forms:  Advisor, Committee Members, Academic Affairs Director.

General

Course Related

  1. PhD Course Requirements Waiver Request / CMS Core Class Waiver Request - Use to document courses from a completed master's degree approved to be counted towards the CMS Ph.D. course requirements, this form is to be completed as early as possible after admission. Note: Courses from Non-Degree or incomplete degrees should be documented using the Graduate Transfer Course Form.
  2. Graduate School Petition * - Use to adjust registration after the drop/add deadline, submit to the CMS Academic Affairs office for processing.

 

Dissertation Related

  1. Dissertation Proposal - SACS Evaluation Form* - Distribute to each of your committee members during your dissertation proposal, submit completed forms to CMS Academic Affairs office directly after thesis proposal, target: year 2.
  2. Committee Appointment * - Use to officially form your committee, submit completed forms to CMS Academic Affairs office, target: year 2.

  4. PhD Comprehensive Exam - SACS Evaluation Form* - Distribute to each committee member before your exam, return completed forms to CMS Academic Affairs office directly after the comprehensive exam.
  5. Admission to Doctoral Candidacy Form* - Submit to the CMS Academic Affairs office for processing directly after completion of all comprehensive exams and other program requirements, once approved you will be able to register for dissertation credits.
  6. Electronic Thesis and Dissertation (ETD) Resource Center - Review the Graduate School ETD website one semester before graduation.

  1. Admission to Doctoral Candidacy Form* - Submit to the CMS Academic Affairs office for processing directly after completion of all comprehensive exams and other program requirements, once approved you will be able to register for dissertation credits.
